First Holy Communion Official Name Form 2022/2023

There are a lot of things that need to be in place for your child in order to make their First Holy Communion. Please complete the full name of your First Holy Communicant candidate child as you would like it to appear on your child’s certificates, Church program etc. Please include any punctuation and or accent marks your child’s name may have. If you have more than one child making their First Holy Communion this year, please fill out a separate form for each child.
